Hey rocker,

You’ve signed up for Rock Camp 2026, hell yeah!

To help us place you in a band and get to know you better, please fill out the questionnaire as accurately as possible.

Medical Form

Do you have any allergies, take medication to camp, or are there other medical matters we should be aware of? Have your parent or guardian fill out the medical form and upload it under medical form. If you are over 18, you may complete and upload the form yourself.

If you prefer not to upload the form online for privacy reasons, please send it to info@derockschool.be or drop it off in person at the reception.

Photo and Video Consent

During our summer camp, many photos and videos are taken each year. We like to use these images to share the fun of our activities via social media, the newsletter, and our website.

By giving your consent at the bottom of this form, you (or your guardian, if you are under 18) acknowledge that you are aware of the possible ways in which the media may be used and shared.

Important: I hereby explicitly, irrevocably, and on a non-exclusive basis transfer all rights regarding the media taken during the summer camp to De Rockschool, with the right to publish and distribute this material through the aforementioned channels.

The information you provide here will be used solely for your registration in our Rockkamp.
